We are unable to accept credit card information on-line for C.S.T. enrollment. Below are listed several options for you to use.
OPTION 1 - PRINT OUT FORM AND MAIL WITH CHECK

- Mail with Application Fee payment of $50.00 to:

OPTION 2 - SUBMITTING ENROLLMENT FORM ON-LINE THEN MAIL APPLICATION PAYMENT WITH CHECK

OPTION 3 - SUBMITTING ENROLLMENT FORM ON-LINE THEN CALL THE ASET OFFICE WITH CREDIT CARD PAYMENT INFORMATION

- Call ASET office with Credit Card information (VISA or MasterCard only)

IMPORTANT:
Upon receipt of your C.S.T. ENROLLMENT FORM and application fee, you will be sent curriculum vitae forms and qualifying questions from the committee. (Application will not be processed until application fee is received.)
When you receive the curriculum vitae forms, please complete, answer the committee questions and return the materials. Upon your acceptance into the program, additional information will be forwarded to you.
